    ADMISSIONS ARRANGEMENTS

    In order to help you understand the admission process better, we have produced a number of documents listed below. Please read through the documents carefully, and we hope you find them helpful and informative.     Admission in-year application forms Reception - Year 6:

    If you wish to apply for a place for your child for entry into another year group or for entry into EYFS after the normal September intake please complete the following forms:

    Declaration of Interest Form for School

    Supplementary Information Form

    Admission Application Pack Reception - Year 6

    Along with the above completed forms, please submit a copy of your child's birth certificate and baptism certificate if applicable.

    Applications for entry to our school should be made through Staffordshire County Council's co-ordinated scheme (Invite to preference letter 2023 Admission).  

    Applications other than the normal intake to reception class (In-year applications):

    You will be required to complete an in-year admissions form which is available on the SCC website:

    Please take this form to your child's current headteacher and ask them to complete Section 4 before submitting your application, which should be submitted directly to St Modwen's.

    Please also complete a declaration of interest form (see below) and return it to school as soon as possible (office@st-modwens.staffs.sch.uk)

    In-Year Applications

    An application can be made for a place for a child at any time outside the admission round and the child will be admitted where there are available places. An application should be made to the school by completing the in-year admissions application form which is available on our website . A hard copy of the application form can be obtained by contacting the school office  01283 247560 and admissions@st-modwens.staffs.sch.uk.

    Completed application forms must be and returned to admissions@st-modwens.staffs.sch.uk

    If there are no places available, the child will be added to the waiting list and further information relating to this will be included in the letter sent from the school advising you of the outcome of your application. Please see the admission arrangements for more details.

    You will be advised of the outcome of your application in writing as soon as possible. Applicants must be informed of the outcome of their application within 15 school days of its receipt, but the aim is to notify applicants of the outcome of their application within 10 school days of its receipt.

    You have the right to appeal to an independent appeal panel if your application is unsuccessful.

    St Modwen's school now has a published admissions number (PAN) of 60. This means that each year there are 60 new places available in the Reception class.
